Debbie C. Cascaden, 63, of Adolphus, KY passed away Monday, May 17, 2021 at Tri-Star Centennial Medical Center in Nashville, TN. The Fremont, OH native was a retired nursing administrator for Signature Health Care in Bowling Green, former regional administrator for Signature Health Care and former nurse and administrator for several local hospitals and nursing facilities. She was a member of St. Joseph Catholic Church in Fremont, OH. She is a daughter of the late William D. Forsyth, Sr. and Marjorie Christine Travis Forsyth and wife of the late John Frederick Cascaden.
She is survived by 1 son: John “J. D.” Cascaden and wife, Shannon, Adolphus, KY;
1 sister: Cindy Ann Rizzardi and husband, John, Fremont, OH;
1 sister-in-law: Patricia Forsyth, Fremont, OH;
3 grandchildren: Misty Cascaden, Hunter John Cascaden and Ally Cascaden;
3 step grandchildren: Seth Parkhurst, Misty Holt and K. J. Holt.
She was preceded in death by a brother: William D. Forsyth, Jr. and a sister: Nancy E. Forsyth.
Funeral service will be 10:00 a.m. Saturday at Pleasant Ridge Church with Bro. Tommy Reels officiating and burial in Pleasant Ridge Cemetery. Goad Funeral Home is in charge of the arrangements.
